caso

Introdução
A Síndrome de Vasoconstrição Cerebral Reversível (SVCR) é caracterizada por cefaleias em trovoada intensas, recorrentes, de início súbito,
associada ou não a sintomas neurológicos focais. Geralmente, é autolimitada, com resolutividade de 1 a 3 meses, mas suas principais
complicações são acidente vascular cerebral isquêmico e hemorrágico. A SVCR é atribuída a desregulação temporária no controle do
tônus vascular cerebral desencadeada por exposição a fármacos vasoativos, presença de tumores secretores de catecolaminas e pode
ocorrer espontaneamente, durante a gestação ou período puerperal.
Material e Métodos
Descrição de relato de caso de Síndrome da Vasoconstrição Cerebral Reversível durante o puerpério.
Resultados
Paciente do sexo feminino, 34 anos, GII PI AI, apresenta, no segundo dia de puerpério, quadro de cefaleia thunderclap, holocraniana,
súbita, persistente, com discreta rigidez nucal terminal, acompanhada de náuseas e vômitos, sem gatilho evidente ou décits neurológicos
focais. Fundoscopia sem alterações. Foram realizadas tomograa computadorizada e angiotomograa computadorizada de crânio
fase arterial e venosa - sem alterações. A coleta liquórica evidenciou aspecto xantocrômico do líquor. Iniciada terapêutica empírica com
Nimodipino, além de analgesia regular (dipirona EV e morna) com melhora parcial de quadro de cefaleia, porém sem resolutividade.
Após 10 dias, realizada arteriograa, que revelou vasoespasmo leve no sistema vertebrobasilar mais evidente no seu terço medial,
conrmando suspeita de SVCR. Não foi evidenciado malformação arteriovenosa ou formação aneurismática. A paciente permaneceu
internada até realização de nova arteriograa, em 14 dias, demonstrando resolução das alterações previamente encontradas. Em
seguida, obteve alta, assintomática.
Conclusão
A SVCR é uma das complicações possíveis do período pós-parto, com dois terços dos casos ocorrendo durante a primeira semana
puerperal. Os pacientes com SVCR devem receber tratamento sintomático com analgésico. A nimodipina é utilizada para reduzir a
intensidade e número de episódios de cefaleia, apesar de não mostrar diminuição do tempo de vasoconstrição. Manifestações como
cefaleias recorrentes em thunderclap nesses casos devem levar o médico assistente a suspeitar de SVCR como um dos diagnósticos
diferenciais e considerar a utilização de exames de imagem como importantes ferramentas para diagnóstico.
